【摘要】目的观察基质金属蛋白酶-7（mmp-7）蛋白在
喉癌中的表达，探讨其表达与肿瘤侵袭转移、血管生成及预后的关系。方法采用免疫组化方法检测mmp-7在48例喉癌标本中的表达水平，运用χ2检验、直线回归分析、乘积极限法、对数秩检验进行统计学分析。结果mmp-7在喉癌中的阳性表达率为81．25%（39/48），明显高于癌旁正常喉黏膜上皮的18．75%（6/48）；mmp-7与患者年龄、肿瘤病理分化程度无关，与喉癌原发灶浸润深度、区域淋巴结转移范围和临床分期相关；喉癌mmp-7蛋白表达水平与肿瘤微血管密度呈线性依存关系，其回归系数为0．60；低表达（阴性或弱阳性）mmp-7的喉癌标本术后3年和5年的生存率分别为82．38%、73．23%，明显高于高表达（阳性或强阳性）mmp-7的喉癌标本术后3年和5年的生存率分别为66．42%、44．28%。结论mmp-7在喉癌中呈阳性表达，其表达与肿瘤侵袭转移程度呈正相关系，与喉癌微血管密度呈线性依存关系，高表达mmp-7的喉癌预后较差。

喉癌是头颈部常见的
恶性肿瘤，较其他头颈部肿瘤，喉癌虽然有较低的转移率，但是局部复发和转移仍为喉癌患者5年内死亡的主要原因[1]。
