PurposeTocomparethehipstrength（hipabductor,externalrotator,andextensor）andlowerextremitykinematics（frontalandtransverseplaneofhipjoint）betweenfemalewithandwithoutpatellofemoralpain,andtoinvestigatetherelationshipbetweenhipstrengthandlowerextremitykinematicstoseeiftheseparameterscouldpredictthefunctionalactivity.

MethodsTwentyfemalesubjectswithpatellofemoralpain,and20matchedhealthycontrolsparticipated.Allsubjectsunderwentmusclestrengthtestingofhipmusclesbyhand-helddynamometer,andlowerextremitykinematicstestingduringfunctionalascendanddescendstairsbyFastrak.Thefunctionalactivitywasmeasuredbyanteriorkneepainscaleandascend/descendstairs.

ResultsFemalesubjectswithpatellofemoralpainshowedsignificantlydecreasedhipexternalrotatorandextensorstrength（P<.05）,andlowerhipabductorstrength（P=.06）comparedtothematchedcontrolgroup.Theyalsodemonstratedmorehipadductionandinternalrotationduringfunctionalascendanddescendstairs（P<.01）comparedtothematchedcontrol.Thefunctionalactivityperformancewassignificantlydifferentbetweenpatientsandcontrols（P<.01）.Therewasnorelationshipbetweenhipstrengthandlowerextremitykinematics（P>.05）,andbetweenhipstrength,lowerextremitykinematics,andanteriorkneepainscale（P>.05）.However,hipextensorstrengthwassignificantlyassociatedwithfunctionalactivity（r=-.34~-.39,P<.03）.Thehipadductionangleduringupstairswascorrelatedwithascendstairsperformance（r=.53,P<.001）,whilethehipinternalrotationangleduringdescendstairswascorrelatedwithdescendstairsperformance（r=.43,P=.005）.Multipleregressionsshowedascendstairstime=5.99–0.04*hipextensorstrength+0.25*hipadductionangle;descendstairstime=3.25–0.04*hipextensorstrength+0.18*hipinternalrotationangle.

ConclusionYoungfemalesubjectswithpatellofemoralpaindemonstrateddecreasedhipexternalrotatorandextensorstrength,andshowedgreaterhipadductionandinternalrotationduringfunctionalascendanddescendstairscomparedtothehealthycontrols.Hipextensorstrength,hipadductionandinternalrotationanglewereassociatedwithascend/descendstairsoffunctionalactivity.文目次口委T-I致x-II中文摘要-III英文摘要-V目-i表目.-iiiD目-vi附件目-v
