RestrictionofDietaryCalories,FatandIronImprovesNon-alcoholicFattyLiverDiseaseMikaYamamotoDepartmentofGastroenterologyandHepatology,DivisionofClinicalMedicineandBiomedicalSciences,InstituteofMedicalScience,MieUniversityGraduateSchoolofMedicineAbstractBackground:Thepathogenesisofnon-alcoholicsteatohepatitis(NASH)isunclear.RecentstudiessuggestedthatoxidativestressplaysanimportantroleinthemechanismofNASH.Excessiveaccumulationofironinthelivercausesoxidativestress.Theaimofthepresentstudywastoevaluatethegradeofhepaticironaccumulationandthetherapeuticresponsetorestrictionofcalories,fatandironinpatientswithnon-alcoholicfattyliverdisease(NAFLD).
Methods:Twenty-sevenNAFLDpatientswereenrolled.Thepatientswerecategorizedintotwogroups:17patientswithNASHand10withsimplesteatosis.TwelveNAFLDpatients(NASH,n=9;simplesteatosis,n=3)weregivenadietaryprescriptionincludingrestrictionofenergy,fatandiron.
Results:Positiveironstainingwasobservedin71%and50%ofpatientswithNASHandsimplesteatosis,respectively.Theaverageenergyintake,fatenergyfractionandironintakedecreasedsignificantly6monthsafterthebeginningofthedietinallpatients.Inaddition,thelevelsofserumtransaminaseandferritinweresignificantlydecreased.
