脑动静脉畸形的影像对比研究

　　【摘要】目的探讨脑内动静脉畸形ct、mri、cta、mra、dsa等影像学诊断及其与病理学表现的相关性。方法本文收集颅内动静脉畸形54例，其中脑动静脉畸形45例，脑膜动静脉畸形1例，硬脑膜动静脉畸形1例，硬脑膜动静脉瘘7例，并经过ct、mri、cta、mra、dsa及手术（病理）明确诊断。结果ct呈团块状、蜂窝状、结节状、条索状、斑片状或斑点状;mr呈蚯蚓状或线样无信号流空血管影;cta、mra、dsa可见引入动脉影及引流静脉影，ct发现钙化及超早期出血较mr敏感。血管造影成像可以反映脑动静脉畸形的大体病理结构。结论不同的影像手段对avm的诊断各具有价值，结合不同的影像手段完全可以达到诊断定性的目的，影像表现可以反映avm大体病理的表现。　　【关键词】脑动静脉畸形影像诊断ctmrictamradsa病理　　imagingdiagnosisofarteriovenousmalformationofbrainandcorrelationofitspathology　　zhengchunyu，litianran，qiangennian，etal.　　departmentofradiologyoffuzhougeneralhospitalofpla，fujian350025.　　【abstract】objectivetodiscussarteriovenousmalformationimagingdisplayofct，mri，cta，mra，dsaanditspathology.methodstocollectionarteriovenousmalformation45casesandconfirmedbyct，mri，cta，mra，dsaandpathologyaftersurgicaloperation.resultsinthect，imagingdisplaymass，honeycomb，nodosity，strip，patchandblotchforms.inthemr，imagingdisplayearthwormorlineformthathavenosignalinthevessels;thereforethecta，mra，dsamayfindtheintroductionarteriesanddrainveins.thectdiscoversescalcifysensitive-lythanmri.theangiographycanreflectthepathologicconstructionofarteriovenousmalformation.conclusionthedifferentimagemeanshaveitsvalueindiagnosisavm.tocombinedifferentimagemeanscancompletelythediagnosisavm.theimageperformancecanreflectthepathologicperformanceofarteriovenousmalformation.　　【keywords】cerebralarteriovenousmalformationcomputerizedtomographicangiographymagneticreso-nanceangiographydiagitalsubtractionangiographypathologyimagingdiagnosiscomputedtomcyraphy　　脑动静脉畸形（arteriovenousmalformation，avm）属先天性中枢神经系统血管发育异常，是颅内最常见的血管畸形。临床主要以自发性脑出血为主要表现，有报道认为50%～69%的avm合并脑出血［1］。影像检查是诊断脑血管畸形最常用的诊断方法。本研究主要分析不同影像手段对avm诊断价值的比较及与病理的相关性。
